New Urgent GP Referral Forms

In response to the publication of the NICE guidance, "Referral Guidance for Suspected Cancer" which was published in June 2005, the Pan Birmingham Cancer Network has developed a generic referral form for each tumour site treated within the Network. All forms comply fully with the NICE guidance and should only be used for suspected cancer and in conjunction with the NICE Referral Guidelines for Suspected Cancer, June 2005.  The Pan Birmingham Cancer Network has developed these forms in conjunction with IT leads from the PCTs served by the Network.
